[이 연구는 재난적 의료비 지출이 가구의 빈곤화에 미치는 직접적인 영향을 파악하고, 그 과정에서 재정적 대응의 매개효과를 검증하고자 수행하였다. ‘한국복지패널’의 7~8차 조사에 참여한 가구 중에서 7차 연도에 빈곤하지 않은 3,321가구를 대상으로 재난적 의료비 지출, 재정적 대응의 변화, 빈곤화 간의 연관성을 파악하기 위하여 매개효과 모형을 적용한 로짓 회귀분석과 매개 효과의 유의성 검정을 실시하였다. 분석 결과, 재난적 의료비 발생 가구는 미발생가구에 비해 빈곤화될 위험이 높았고, 사적이전 소득과 민간보험 수령액이 유의하게 증가하고, 재산은 유의하게 감소하였다. 가구의 재정적 중에서 사적이전 소득·저축액·비-의료서비스 소비지출의 증가는 가구 빈곤화와 음(-)의 관련성이 있었고, 이 과정에서는 사적 이전 소득만이 매개 효과가 있었다. 그러나 사적이전 소득과 같은 비공식적 대응만으로는 의료비로 인한 가구의 빈곤을 완화시키는 데 한계가 있다. 따라서 의료비로부터 경제적으로 취약한 가구를 보호하기 위해서는 의료 보장성 확대와 본인부담금 경감제도와 같은 공적 지원제도의 강화가 필요하다.,
This study aimed to examine the direct effects of catastrophic health expenditure on the transition to poverty of households in South Korea, to verify the mediating effects of financial coping in the causal process. This study involved 3,321 households who participated in the 7th-8th KWPS and were surveyedas non-poor households in the 7th. To examine catastrophic health expenditure in these households, changes in financial coping, and the association with transition to poverty, a three-step logistic regression were performed. The results showed that the households facing catastrophic health expenditure had a significantly higher risk of facing poverty than the households without catastrophic health expenditure, and the households facing catastrophic health expenditure showed a significant increase in private transfer income and private insurance benefits whereas they showed a significant decrease in assets. Between households’ financial coping and transition to poverty, increased private transfer income, increased savings, and increased non-healthcare service spending were negatively(-) associated with the transition to poverty of households. In addition, only private transfer income had a mediating effect in the process of catastrophic health expenditure affecting the transition to poverty of households.]
