아동 주치의등록제에 대한 정의론적 접근

초록[abstracts]
[서울시를 시작으로 몇몇 지역자치단체는 아동 주치의등록제를 도입하고 있으며, 그 구체적인 안으로 서울시가 시행하고 있는 아동 · 청소년 치과주치의 제도와 우리아이 건강관리의사 제도가 있다. 두 사업은 일차의료 확대, 의료비용 조절, 건강불평등 완화 등을 목표로 하여 시행되었으며 초기 시범 사업 이후 점차 대상을 확대하고 있다. 특히 치과주치의 제도의 경우 저소득층 아동에 심화처치를 제공하여 대상 집단의 우식 예방 효과가 있음을 보였다. 이 사업에 대해 서울시 치과의사회와 소아청소년과 개원의사회 서울지회는 반대 의사를 밝힌 바 있으며 그 이유로 사업 과정에 관한 협의 부족, 재정 불충분, 제공의료 서비스의 질 하락, 과도한 행정 부담 등을 제시하였다. 여기에선 아동 주치의등록제가 내세운 목표가 현재 구현된 제도와 일치하지 않으며 반대 의견 또한 부분적으로 타당하여 현재 제도가 지속성과 정당성을 담보할 수 없음을 살피고, 대신 공정성에 기반을 두어 아동 주치의등록제의 타당성을 확보하려 시도하였다. 이를 위해 의료정의의 세 입장, 공리주의, 약자우선주의, 운 평등주의를 개괄하고 각 입장에 비추어 아동 주치의등록제를 살핀 뒤, 공정한 아동 주치의등록제 시행을 위한 개선점을 제시하였다.
Several local governments in South Korea introduce a registered doctor system for children, which have been started from Seoul. Two proposals, the Registered Dentist System for Children and Adolescent and the Health Management Doctor System for Children, are implemented by the Seoul government. The primary objectives of these services are an expansion of primary health care, control of health care cost, and mitigation of health inequality. The services have been expanded after the pilot projects had completed. The Seoul Dental Association and the Korean Association of Pediatric Practitioners oppose these services because of lack of consultation with the professional community, financial limitation, degradation of health service quality, and excessive administrative burdens. This paper reviews the implemented policies and argues that there is a gap between the objectives of the registered doctor system and the aims of the implemented policies. In effect, the current services could not guarantee sustainability and legitimacy of the policy. Therefore, this paper tries to secure the validity of the registered doctor system for children based on the justice. For this purpose, three main positions of the health justice are introduced and suggestions for improvement of the just registered doctor system for children are made.]
