초록[abstracts]
[본 논문에서는 생명의 마지막과 관련된 윤리문제를 기독교적 세계관으로 조명해보고자 한다. 특히 최근 시행된 연명의료결정법을 중심으로 이를 어떻게 이해하며 기독교인으로서 어떤 자세를 가져야할지 통합적인 관점에서 풀어보고자 한다. 과연 죽음이 무엇인지, 죽음의 정의에 관해 심폐사, 뇌사, 세포사 등을 살펴보고, 필자는 불가역적인 심폐정지를 죽음의 시점으로 보는 것이 옳음을 제시한다. 아울러 뇌사를 기독교적 세계관의 관점에서 죽음의 기준으로 받아들일 수 있을지 신중한 검토가 필요하다고 본다. 아울러 안락사는 그 어떠한 형태로도 허용될 수 없으며, 이와 구분되는 연명의료 중단은 돌이킬 수 없는 임종과정에서 엄격한 기준 하에 예외적으로 받아들여질 수 있다. 올해 초 시행된 연명의료결정법의 입법과정과 주요 내용을 살펴보면 사전연명의료의향서와 연명의료계획서 작성이 본인의 결정을 확인하기 위해 무척 중요하며, 법안의 쟁점인 자기결정권과 연명의료중단 대상의 범위와 확대에 대해서는 매우 신중하게 적용해야 할 것이다. 아울러 기독교가 앞장서 호스피스 운동을 활성화하고 연명의료결정법 이후 다양해지는 죽음문화에서 부활의 종교인 기독교가 주도적으로 죽음준비교육을 선도하며 죽음을 앞당기려는 생명경시풍조에 맞서 생명존중문화를 이끌어가야 할 것이다.
The purpose of this study is to discuss ethical issues related to end-of-life care and to explain the Life-Sustaining-Treatment Decision-Making Act from an integrative Christian perspective. Three definitions of death, hereby categorized as cardio-pulmonary death, brain death and cellular death, can lead to different views and potentially different policy decisions. The author believes that associating the point of death with an irreversible cardio-pulmonary failure is consistent with the Christian beliefs, whereas brain death theory is still under debate and scrutiny to be accepted from a biblical perspective. Euthanasia should not be allowed, but removal of life sustaining treatments should be understood from a separate exceptional case, given strict standards for the determination of irreversibility. ‘Life-Sustaining-Treatment Decision-Making Act’ was legislated and enforced on 4 February 2018. Both the Advance Directive and POLST are critical to the confirmation of self-determination, thence the scope of legal self-determination should be cautiously inspected and corrected, as well as the application and enforcement in clinical fields. Moreover, Christians should accordingly lead the hospice movement and develop well-dying education that thrives the sociocultural value and respect for life.]
