현대의학의 '개별 맞춤 의학'에 대한 비판적 고찰

초록[abstracts]
['Personalized medicine(PM)', cutting-edge therapy of modern medicine propelled by rapid progress in medical knowledge and scientific technology, is showing outstanding therapeutic effects by trying disease treatment for specific targets reflecting each patient's individual characteristics and each disease status. However, PM based on biomedical models has many problems and may induce new problems. PM, which treats the disease by aiming at specific targets in an aspect of biomedicine, could make problems by ignoring possible complex interactions between many factors except the targets. For these problems, 'general therapy' considered only for symptomatic alleviation needs to be noticed and utilized more. From the aspect of philosophy of medicine, the problems are related with the human desires facing PM. Too much confidence on therapeutic technology of modern medicine like PM may make each patient have excessive desires beyond the level of disease treatment, thus inducing indiscreet medical commercialization by any possibility. Optimal balancing between expression and control of desires regarding disease treatment is therefore required. Contrary above problems, the subjective and intrinsic sensibility or existential desires of each patient undergoing the process of experiencing diseases are not nearly fulfilled. This derives from the limitation of biomedicine tailor-making only the biological level of each patient. Efforts to approach the subjective realm of each patient experiencing the process of disease treatment is required. In addition, PM could induce medical-economic problem of very high medical expenses. Although PM recently has much spotlight in modern medicine, it does not still reach the level of medicine tailored to fit the subjective and particular desires of each individual patient beyond the biological level. Therefore, multidisciplinary approach is acutely required to substantialize a bona fide PM.
의학지식과 과학기술의 급속한 발전에 힘입어 현대의학의 첨단적 치료요법인 '개별 맞춤 의학'은 환자 개개인과 개개 질병의 특성을 반영하는 특정 표적을 대상으로 질병 치료를 시도하여 우수한 치료효과를 나타내고 있다. 그러나 생의학적 체계에 기반을 둔 '개별 맞춤 의학'은 많은 장점에도 불구하고 여러 문제점을 안고 있으며 앞으로도 새로운 문제점을 초래할 수 있다. 생의학적 측면에서 '개별 맞춤 의학'은 특정 유전학적 또는 여타 특정 표적만을 겨냥하여 치료하므로, 이들 표적 이외 여러 인자들의 복합적인 작용 가능성을 간과하고 배제함에 따른 문제점이 발생하고 있다. 이를 위해 질병의 증상 완화에만 그친다는 부정적 인식이 지배적인 '일반적인 치료요법'을 주목하고 더욱 활용할 필요가 있다. '개별 맞춤 의학'을 대하는 인간의 욕구와 관련된 의철학적 측면에서, 현대의학의 치료기술에 대한 지나친 확신으로 인하여 질병치료 수준을 뛰어 넘는 환자 개개인의 과도한 욕구는 자칫 무분별한 의료상품화와 상업적 의학의 문제를 파생할 수 있다. 따라서 질병치료에 대한 욕구의 표출과 자제 사이에 적정한 균형이 필요하다. 이와는 상반되게 질병체험과정에서 환자 개개인이 겪는 주관적이고 고유한 감성과 내밀한 실존적 욕구는 거의 충족되지 못하고 있다. 이는 환자 개개인의 생물학적 수준에만 맞추어주는 생의학의 한계에서 비롯되므로, 질병이 치료과정에서 직접 체험하고 있는 환자 자신만의 주관적인 영역에도 다가가서 실제 도움을 주기 위한 노력과 연구가 요구된다. 이밖에도 '개별 맞춤 의학'은 의료비용의 상승이라는 의료경제적인 문제점을 지니고 있다. 비록 우수한 치료효과를 나타내는 '개별 맞춤 의학'이 현대의학에서 최근 각광을 받고 있다할지라도 생물학적 수준을 넘어 환자 개개인의 주관적이고 고유한 욕구까지도 맞추어 줄 수 있는 보다 온전한 맞춤 의학의 단계에는 아직 도달하지 못하고 있다. 따라서 보다 실질적인 '개별 맞춤 의학'의 실현을 위해서는 관련 분야들의 다학제적 접근이 절실히 요구된다.]
