초록[abstracts]
본 논문은 호스피스 병동에서 봉사하는 재가신도가 환자 생존부터 사망까지 전인적 돌봄을 하는 과정에서 관찰한 실제의 경험을 토대로 한다. 거기에‘상구보리 하화중생’하는 대승보살정신과 불교교리를 더하여 환자와 가족의 이익과 봉사자의 보살수행에 이익이 되는 방법을 일관성 있게 정리하고 공유할 필요를 느껴왔다. 하지만, 수많은 중생들의 근기에 따라 다양한 삶의 과정에서 생긴 고통해결을 위한 부처님의 방편설과 그것을 해석한 교리는 너무나 방대하고 복잡하여서 시한부 삶을 사는 환자에게 쉽게 전달하기 어려웠다. 또, 봉사자 여럿이 다른 날짜에 봉사하므로, 제시하는 내용에 일관성이 없어서 환자가 혼란스러워 하거나 믿지 않았다.
여러 봉사자가 각각 다른 날짜에 오더라도 한 사람이 돌봄 하는 것처럼 통일성을 느껴서 환자와 가족들이 믿고 함께 할 수 있는 방법을 찾았다.
그것은 죽어가는 환자의 제한된 기간에 맞춰 불교가 가진 수많은 경전과 거기서 나오는 여러 귀의 대상을 과감하게 일원화하고 단순화하게 표준화하기위해 매뉴얼을 작성하기로 하였다.
그 결과 돌봄을 크게 4단계로 나누고,
첫 번째, 환자가 직면한 죽음의 고통에서 벗어나게 하기 위해 현대 심리학과 유식학의 방어기제를 활용해 환자의 핵심감정을 관찰하여 제시 한다.
두 번째, 죽음을 수용하여 우울해하는 환자에게 (자신의 고통 자각하는지-고통 원인 아는지- 고통 해소하고 싶은지 - 물어보고 해소방법을 제시) 부처님 가르침을 설명하고‘아미타불’의 극락을 이해시킨다. 죽음의 고통과 번뇌를 멈추게 하는 대안으로 사성제와 팔정도를 제시하여 귀의케 하고 육바라밀행의 보시 지계 등을 지키며 남은 생을 올바른 삶이 되도록 권한다.
세 번째, 환자가 자신의 수행차제에 따라 안정된 마음을 유지하며‘참회와 화해’, ‘수용과 용서’하여 금생의 인간관계를 잘 정리 하고 내생에 선처에 나기위해 자비도량 참법, 육바라밀수행 등을 통해 유산정리와 보시 등을 위한 유언장을 작성한다. 또, 연명의료 의향서 작성을 통해 마지막순간 신체적 고통의 원인을 제거하고 평온함을 유지 하도록 권유한다.
네 번째, 현재의 삶을 정리하고 ‘아름다운 마무리’를 위해 사회에 기여하는 보살행 실천 속에서 임종을 맞아서 ‘아미타불’십념을 성취하는 것을 최종 목표로 2014년 6월에 초안을 작성하였다.
초안은 봉사자들의 의견과 전문가들의 십여 차례 꾸준한 조언을 받아 수정을 거듭하여 현재의 매뉴얼을 완성하였다. 향후 임상 활동가들의 피드백을 통해 꾸준히 개선 보완해 나갈 것이다. 본고가 재가 불교호스피스 활동에 미력하나마 도움이 되길 바란다.

Reality of Upasaka and Upasika’s caring volunteer service
This thesis is based on real experiences in process of upasaka and upasika who volunteer works at hospice unit serving total care service to patient from survival to death.
In addition feel to need organize consistency and share beneficial method to patient and his family’s profit and bodhisattva practicing with adding the great vehicle-bodhisattva sprit, ‘SANGGUBORI HAHWA JUNGSAENG’, and Buddhism creed.
But it is hard to deliver expedient means (methods to solve cessation of many living beings followed by endurance) and its creed to terminally ill patient because those are so huge and complexity. And different each volunteers serve different day, their contents are not consistent so patients are confused or don’t believe them.
I found the way to patient and his family can believe and do together, even each volunteers visit different day but feel having unity
That is tuning dying patient’s limited period, writing this manual for make number of Buddhist scriptures and their devotion targets unification and simply standardization.
Consequently, caring is divided into 4 steps.
First, for escaping from facing cessation of death, using the modern science of psychology and yogacara thought’s defense mechanism, observe patient’s core feeling.
Second, explain the meaning the teachings of Buddha and understanding pure land of Amitabha to depressed patient who accept his death. Alternative of stopping death’s cessation and defilements, suggest the four noble truth (suffering, cause of suffering, cessation, the path of cessation) and noble eightfold path (right view, right thought, right speech, right action, right livelihood, right effort, right mindfulness, right concentration), recommend right way to live through make patients are converted to Buddhism and following six perfections charity and moral.
Third, a patient keep stable his mind with bodhisattva practicing, well organize relationship in this life as doing ‘repentances and reconciliation’, ‘accept and forgiveness’ and make a will through compassion-generous, repentance with Buddhist scriptures and fulfilling six perfection for favorable in next life. As writing letter of intent medical care for life prolongation, remove physical cause of suffering and recommend keep calm.
Fourth, in Bodhisattvas practice, sorting current life and contributing to society for ‘beautiful completion’, final goal is achieving tenfold prayers. This draft was written June, 2014.
This draft is revised with volunteers and received expert’s advice numerous times, current manual is completed. From now on, the draft will be compensated with clinical activist’s feedback. Hope this thesis can be useful for JAEGA Buddhism hospice activities.
