호스피스 팀의 호스피스 및 연명의료결정참여 경험에 대한 사례연구

국문 초록[abstracts]
본 연구 목적은 호스피스 및 연명의료결정 과정에 참여한 호스피스 팀의 상호관계를 분석하여 말기환자와 가족의 결정을 지원할 수 있는 방안을 논의하는 것이다. 호스피스팀의 경험을 분석하기 위하여 의사, 간호사, 사회복지사 등 9명을 대상으로 개방질문지를 사용하여 결정시작 시점, 주 결정자, 결정내용들을 개별, 심층 면접하였다. 사례연구방법을 이용하여 중심개념을 분석한 결과, 8개의 범주와 19개 하위범주, 86개 개념들을 도출하였다. 사례 간 의미와 주제를 분석한 결과 첫째, 참여자들은 결정시작 시점을 암 전문 의사의 치료중단, 퇴원권유 시점으로 경험하였으며, 둘째, 주 결정자는 환자보다는 가족들로 경험하였다. 셋째, 환자와 가족은 무엇을 결정해야하는지 혼란스러워 하였으며, 호스피스 및 연명의료 외에도 장례절차, 임종준비, 경제지원 여부를 결정한다고 하였다. 결정은 한 번에 하기 보다는 시간을 두고 하는 과정으로 이해하였으며, 호스피스 팀의 지속적 지원이 필요함을 강조하였다. 마지막으로 새로운 법에서 호스피스를 이용할 수 있는 환자의 조건이 엄격하여 또는 서비스가 부족하여 결정을 자유롭게 하지 못하는 경우도 있다고 하였다. 본 연구는 호스피스 협진체계 정비, 생애주기별 죽음교육실시, 호스피스 팀 활동 보장 및 환자권익 보호를 위한 법 개정을 위한 정책적·제도적·실천적 지원 방안에 대하여 논의하고자 한다.
다국어 초록[Multilingual Abstract]
This study aims to analyze hospice teams’ interrelationship in the patients’ decision of hospice and life sustaining treatments. ePersonal, in-depth interviews were conducted of a total of 9 individuals from hospice team, including physicians, nurses, social workers with three open ended questionaries for the initiating point of decision making, primary decision maker, and contents of decision making. With the case study method, 86 concepts, 19 sub-categories, and 8 categories were derived. This study found the participants experienced that the decision was initiated by Oncologists’ discharge order after stop the treatments. And they experienced primary decision maker was the family rather than the patient. They also experienced it was confused what decisions made for. It was not one time process to consider patients’ medical and nonmedical matters such as life sustaining treatment, funeral plan, caring for dying, financial plan. They found the importance of their engagement to facilitate the decision between the patient and family members. Finally, there were limited options to make decisions by the restricted condition of the patient’s admission and lack of hospice services under the new Act. This study discussed the systematic, policy-wise, and implementation plans that could confirm of the consulting system, death education from life course perspective, hospice team engagement, and the amendment procedure to promote patients’ rights.
