연구배경 장기이식은 말기의 심장, 신장, 간 및 폐질환을 앓고 있는 환자에게서 생명을 구하는 중요한 치료방법이다. 미국에서는 장기이식 환자에 대한 약사의 중재 서비스가 복약이행을 높이고 장기이식 후 이식편의 생존율에 긍정적인 영향을 미친다고 평가되고 있다. 본 연구는 국내 단일 의료기관에서 약사의 중재 서비스가 장기이식 환자와 간호사에 미치는 영향을 평가하였다. 연구방법 본 연구는 대조 전후 연구(controlled before and after study)로 설계되었다. 본 연구의 중재는 수술 후 1개월 동안 약사가 실시한 2회의 복약상담 및 교육으로 이루어졌다. 환자 시험군은 신장이식 교육군(n=34), 환자 대조군은 간이식 비교육군(n=18) 및 신이식 비교육군(n=34)이 참여하였다. 환자 만족도 및 이해도를 자기기입식 설문지를 활용하여 조사하였고, 전자의무기록 상의 혈청 크레아티닌을 후향적으로 비교하였다. 부가적으로, 간호사 인식 변화를 자기기입식 설문지를 활용하여 조사하였다. 장기이식병동 간호사(n=19)에 대해 중재 전후를 비교하였다. 환자 만족도 및 이해도는 t-test와 Fisher’s exact test, 간호사 인식변화는 paired t-test로 비교하였다. 연구결과 복약지도에 대한 환자의 만족도는 중재 후에 증가하였다(p=0.004). 복약지도에 대한 이해도 중에서 의약품명(p=0.053)은 중재 후의 상승이 40%로 상대적으로 크지 않은 반면 의약품효능(p=0.012), 복용법(p=0.026), 부작용(p=0.001), 약물상호작용(p=0.001)은 교육군이 비교육군에 비해 중재 전보다 200~400%의 뚜렷한 이해도의 상승을 보였다. 음식물과의 상호작용으로 인한 혈중농도 지식 또한 비교육군에 비해 높은 정답율을 보였다(p<0.001). 혈청 크레아티닌(mg/dl)은 신장이식 교육군(1.32→1.00)이 비교육군(1.29→1.22)에 비해 약 4.6배 감소하였다(p=0.008). 복약지도에 대한 간호사의 만족도(p<0.001)와 간호사가 평가한 환자의 만족도(p=0.001)는 중재 전에 비해 각각 42%와 26% 상승하였다. 반면 약사의 필요성은 중재 전ㆍ후에 유의한 차이는 없었다(p=0.297). 투약설명 부담 정도와 간호업무 도움 정도에 대해서도 중재 전ㆍ후에 유의한 차이가 나타나지 않았다(p>0.05). 결론 본 연구는 약사의 복약상담 및 교육으로 부작용, 약물간 상호작용, 음식물과 상호작용으로 인한 혈중농도 지식 등 심층적인 지식에 대한 이해가 높아질 수 있음을 제시하였다. 또한, 복약지도를 통한 질병 및 약물지식 향상이 신장기능 향상에 도움이 될 수 있음도 간접적으로 확인하였다. 그러나 간호사의 업무부담을 줄이기 위해서는 보다 정기적인 약료서비스가 필요할 것으로 판단된다.
초록[abstracts]
Background: Organ transplantation is an important therapeutic option for patients with end-stages of heart, kidney, liver and lung diseases. In the US, it is considered that effective pharmacist services could result in affirmative effects on medication adherence and a graft survival rate among post-transplant patients. The study aimed to evaluate the impacts of a pharmacy service on post-transplant patients and the nurses’ work load in a single medical institution. Method: A controlled before and after study was performed. Pharmacy service (intervention) was comprised of twice medication consultants and educations in one month. Intervention group included 34 kidney transplantation patients with pharmacy service and control group included 18 liver transplantation patients with usual care. Changes in patients’ satisfaction and medication knowledge after intervention were investigated using a self-administrated questionnaire. Serum creatinine between intervention and control groups was retrospectively compared employing electronic medical record. Additionally, changes in nurses’ perception were investigated using a self-administrated questionnaire in 19 nurses of the kidney transplantation unit. Patients’ outcomes were compared using t-tests or Fisher’s exact test. Nurses’ outcomes were compared using paired t-tests. Results: The level of satisfaction in patients on medication counseling was increased after intervention (p=0.004). The level of appreciation in patients was increased by 40%, which was relatively small. The levels of appreciation regarding drug efficacy (p=0.012), administration methods (p=0.026), adverse drug effects (p=0.001), and drug/drug interactions (p=0.001) were 200 ~ 400 % higher in intervention group than in control group. The correct rate in the question over blood concentration due to food/immunosuppressant interaction was higher in intervention group than in control group (p<0.001). Serum creatinine was about 4.6 times lower in intervention group (1.32→1.00) than in control group (1.29→1.22) (p=0.008). The level of nurse satisfaction with medication counseling (p<0.001), and the level of patient satisfaction assessed by nurses (p=0.001) were increased by 42% and 26%, respectively. On the contrary, no significant improvement in the nurses’ perspective about whether the counseling pharmacists would be helpful was observed (p=0.297). In addition, the pharmacist intervention of the present study made few changes in the nurses’ work load relating to medications (p>0.05). Conclusion: Pharmacist’s intervention could enhance patients’ in-depth knowledge about medications, such as adverse drug effects, drug/drug interaction, and food/drug interaction. Better knowledge for disease and medications through pharmacist’s intervention may contribute to the improvement of renal function implying a positive prognosis of disease. However, more regular intervention might be helpful to reduce nurse’s work load relating to medications.
