초록[abstracts]
소셜 미디어 상의 빅데이터를 통해 국민의 직관적인 관심도를 파악하여 정부의 기존 정책과의 조화를 이루는 합리적이고 국내 실정에 알맞은 치매 관련 헬스케어 서비스 및 대응전략 수립 활용에 기여하고자 연구를 진행하였다.
연구 대상은 보건의료 관련 정부기관의 문헌고찰과 국내 구글 트렌드 검색을 통해 선정하였고, 이를 통한 각각의 키워드들을 파싱 방식의 웹 크롤링을 통하여 원자료로 수집하였다. 또한 데이터 마이닝을 통하여 네이버의 블로그, 카페, 뉴스의 플랫폼 간 2015년 2월 1일부터 2018년 2월 28일까지의 일별 언급량을 분석하였다.
연구 방법으로는 키워드의 일반적 특성인 빈도분석, 감성분석을 진행하였고 키워드의 언급량을 연도 및 기간별로 비교하였으며, 키워드 간의 상관성을 분석, 치애의 언급량에 영향을 미치는 요인 파악, 치매국가책임제의 언급 여부에 영향을 미치는 요인을 파악하여 최적의 국내 치매 대응 전략 및 정책을 도출하였다.
본 연구는 다음의 결과를 도출하였다.
우선, 빈도분석에서는 키워드 치매예방이 핵심 키워드 치매를 제외하고 가장 높은 빈도를 보였고, 감성분석 또한 키워드 치매예방이 가장 높은 긍정의 값을 나타냈다. 평균비교분석 중 ANOVA를 통해 타 연도와의 언급량 차이를 비교하였고, 독립표본 t검정에서는 치매예방이 정책 발표 이후 가장 많은 언급량 차이를 보였으며, 상관관계분석을 통해 각 치매 관련 키워드 언급량들의 상관성을 확인하였다. 종속변수 치매의 언급량에 대한 위계적 회귀분석을 시행한 결과, 다중공선성으로 요양병원과 노인의료복지시설이 제외되었고 노인장기요양보험, 노인장기요양서비스, 중앙치매센터, 치매안심센터, 치매예방, 치매치료의 6개 키워드가 종속변수 치매에 대한 양의 유의한 영향을 보였으며, 노인돌봄서비스는 음의 유의한 영향을 보였다. 종속변수 치매국가책임제의 언급 여부에 대한 로지스틱 회귀분석과 의사결정트리를 분석한 결과, 두 분석 모두 치매안심센터의 언급량이 높아질수록 종속변수의 언급량 또한 증가하는 것으로 나타났다.
치매관리를 위한 센터 및 기관의 유기적인 네트워크 조성과 지역별 사례관리가 이루어져야 하고 노인장기요양보험 및 서비스와 치매등록통계사업, 치매상담전화센터 등의 홍보와 접근성 제고를 통한 정책의 활성화를 모색하여야 하며, 치매치료에서 치매예방으로 바뀌는 패러다임의 변화에 적극적인 자세가 필요하고 요양병원 및 노인의료복지시설의 부정적 이미지 개선, 재가노인복지시설의 활성화 등 국민의 관심도가 반영된 국내 치매대응전략 및 정책에 대한 전략 수립이 필요할 것으로 사료된다.
초록[abstracts]
The study was conducted to contribute to the utilization of health care services related to dementia and the identification of countermeasures for dementia appropriate to domestic situation harmonizing with the government's existing policies by identifying the intuitive interest of the people through the big data on social media.
The subjects of this study were selected through the review of the literature of government medical institutions related to health care and Google Trend Search. Through this, keywords were collected through parsing style web crawling method as raw data. In addition, through data mining, daily referrals on Naver blog, cafe, and news platform from February 1, 2015 to February 28, 2018 were analyzed.
Frequency analysis and emotional analysis, which are general characteristics of the keywords, were conducted. The frequency of keywords mentioned in the materials was compared by year and by period. The correlations among the keywords were analyzed to identify the factors affecting the frequency that dementia is mentioned, and the factors influencing whether the ‘Dementia National Responsibility System’ is mentioned were identified to induce the most optimal dementia strategy and policy.
This study induced the following results.
First, in frequency analysis, the keyword ‘dementia prevention’ showed the highest frequency except for the core keyword ‘dementia’, and in the emotional analysis the keyword ‘dementia prevention’ also showed the highest positive value. In the mean comparative analysis, ANOVA was used to compare the differences in frequency of mentioning from other years. In the independent sample t test, the keyword ‘dementia prevention’ showed the most significant difference in the frequency of mentioning after the announcement of the policy. The correlation of frequency of mentioning dementia related keywords were identified through correlation analysis. In the hierarchical regression analysis of frequency of mentioning dementia, which is a dependent variable, the nursing hospitals and elderly medical welfare facilities were excluded because of multi-collinearity. The six keywords of long-term care insurance for the elderly, long-term care services for the elderly, central dementia center, complete dementia care center, prevention of dementia and treatment for dementia showed significant positive effects on dementia, the dependent variable. The elderly care service showed a significant negative impact. When a logistic regression analysis and decision tree analysis of whether the dependent variable ‘Dementia National Responsibility System’ is mentioned, both analyses showed that the more frequently ‘Dementia’ is mentioned, the more frequently the dependent variable is mentioned.
It is expected to organize an organic network of centers and institutions for dementia management, to establish case management for each region, to seek promotion of policies through publicity and accessibility of long-term care insurance and services for the elderly, to take a positive attitude of policy change in accordance with the change of the paradigm that changes from the dementia treatment to the dementia prevention, and to establish a strategy for domestic dementia response strategy and policy that reflects the interest of the public by improving the negative image of the nursing hospital and medical welfare facilities for the elderly and activating the welfare facilities for the elderly.
