노인의 임종기치료 선호도 영향요인 – 죽음준비도, 죽음불안을 중심으로
= Influencing factors on the Elderly's Preference in care near the end of life – focusing on preparation for death and anxiety for death

This study was performed to identify the relationship between death anxiety, death preparation, and preference in care near the end of life among the elderly in the community and provide for influencing factors of the preference in care near the end of life . 204 elderly people living in Seoul participated for this descriptive study during Sep. 11th, 2018 to Oct. 12th, 2018 and analyzed by stepwise regression methods using SPSS 23.0. The analysis found that the family decision factors were the highest among the care near the end of life preferences, followed by pain factors, decision-making by the medical personnel, and spiritual factors. On the other hand, the least autonomous decision factors were found to have the lowest preference for life-sustaining treatment. Age, bereavement, marital status, preparation of death, death anxiety, and good income level have important effects on autonomous decision-making factors. The explanatory power of such variables was 37.6 %. The decision-making factors by family members include death preparation, death anxiety, marriage and bereavement. The explanatory power of following variables was 52.1%. In deciding factors by medical personnel, preparation of death, anxiety of death, bereavement and marital status were important factors, and the explanatory power was 42.4%. Therefore, the results indicate the significance of nursing care and the need to take care after elderly when it comes to death by considering individuals’marital status, income levels, and by developing an efficient and systematic death preparation education program , especially those with low preparedness for death and high anxiety
국문 초록[abstracts]
본 연구는 지역사회 노인들의 죽음불안, 죽음준비도 및 임종기치료 선호도와의 관계를 파악하여 임종기치료 선호도에 미치는 영향요인을 규명하기 위해 시도되었다. 서울시에 거주하고 있는 60세 이상 노인 204명을 대상으로 2018년 9월11일 부터 10월12일 까지 자료 수집한 서술적 조사연구로 SPSS 23.0 프로그램으로 단계적 회귀분석을 하였다. 분석 결과 임종기 치료 선호도 중 가족 결정요인이 가장 높고 통증요인, 의료인에 의한 의사 결정, 영성요인 순 이었고 자율적 의사 결정요인이 가장 낮아 연명치료에 대한 선호도가 낮은 것으로 확인되었다. 자율적 의사 결정요인에는 나이, 사별, 기혼상태, 죽음 준비도, 죽음불안, 좋은 소득수준이 중요한 영향을 미치며 이들 변수에 의한 설명력은 37.6%이었다. 가족에 의한 의사 결정요인에는 죽음 준비도, 죽음 불안, 기혼, 사별이 중요한 영향요인이며 설명력은 52.1%이었다. 의료인에 의한 결정 요인에는 죽음 준비도, 죽음불안, 사별, 기혼상태가 중요한 영향요인이었고 설명력은 42.4%이었다. 따라서 이러한 결과에 따라 결혼상태, 소득수준을 고려하고 특히 죽음 준비도가 낮고 죽음 불안이 높은 고령의 노인을 대상으로 임종기 치료 결정을 돕도록 효율적이고 체계화된 죽음준비 교육 프로그램의 개발하여 임종기 치료 준비에 도움이 되어야 할 것이다.
