장애인 가족 돌봄부담의 우울과 자살생각의 관계 연구

초록[abstracts]
The purpose of this study is to examine care burdens of family caregivers caring an adult with physical disabilities or brain lesions and to investigate the relation among the subjectve care burden, depression and suicidal ideation of family caregivers. And positiveness of family relationship(TSL) and social support for caring persons with disabilities have an buffeing effect on the relation among the subjectve care burden, depression and suicidal ideation of family caregivers. The survey was conducted for family caregivers of service users of community rehabilitation center or Independent living center and students enrolled in special schools located in Seoul and Gyeonggi-do province from May to October in 2017 and total 174 were used for final analysis. IBM SPSS 21.0 and Mplus 8 base program were used for statistical analysis.
Major findings of this study are as follows.
First, Objectvie care burden of family caregivers caring an adult with disabilities is as follows. 55.7% of family caregivers responded duration of caring is over 16 years and 63% responded average caring times is over 8 hours in a day, 83.6% responded average caring days is over 6 days in a week. These results means family caregivers have taken care of persons with disabilities over a long period time and restless.
Subjective care burden of family caregivers are measured by caregiving reaction assessment and the avarage score is 1.74 in a range of 1-3. Subarea in care burden are as follows. 'disturbance in my affairs' is 2.15, 'decline in health' is 1.80, 'economic hardship' is 1.76, 'lack of family support' is 1.27.
Second, depression of family caregivers are measured by CES-D(Center for Epidemiologic Studies Depression Scale) and the average score is 20.2(A score of 16 or higher has been used as the cut-off point for depressive symptoms on this scale). Females(20.7) have a higher CES-D score than males(18.0).
24.9% of family caregivers have suicidal ideation in last year. Suicidal ideation among female family caregivers is 26.4% and male family caregivers is 17.2%.
Third, the results of the hypothesis test are as follows.
Test of hypothesis 1: Subjective care burden of family caregivers has a significant effect on suicidal ideation through mediation process with depression.
Test of hypothesis 2-1: Positiveness of family relationship(TSL) of family caregivers dosen't play a significant mediating role in the relation between subjective care burden of family caregivers and depression.
Test of hypothesis 2-2: Subjective care burden of family caregivers dosen't have a significant effect on suicidal ideation through mediation process with positiveness of family relationship and depression (a three-path mediation).
Test of hypothesis 3: Social support for caring of persons with disabilities has a moderating effect on the single mediating relation among the subjective care burden of family caregivers, depression and suicidal ideation (a moderated mediation).
By using structural equation modeling, it is suggested a model of hypothesis 2-2. Pathways are summarized as follows.
[Pathway 1] Depression of family caregivers → Suicidal ideation of family caregivers (+)
[Pathway 2] Subjective care burden of family caregivers → Depression of family caregivers (+)
[Pathway 3] Positiveness of family relationship → Depression of family caregivers (-)
A negative effect of care burden is verified by the significant relation among the subjective care burden of family caregivers, depression and suicidal ideation. Positiveness of family relationship has a significant effect on decreasing the risk of depression though it does not play a mediating role in the relation between care burden of family caregivers and depression. Care services and medical visit services has a significant effect on decreasing the risk of depression through moderated mediation role in the relation among care burden of family caregivers, depression and suicidal ideation.
Practical and policy implications for intervention strategies to support persons with disabilities and family caregivers are discussed. In practical implication, development of mental health improvement program for family caregivers and support program to enrich quality of family relationships for families caring persons with disabilities are suggested. In policy implication, increase support of government to provide care services for persons with disabilities and respite services for family caregivers caring persons with disabilities are suggested.
초록[abstracts]
본 연구는 일상생활에서 지원이 필요한 지체 및 뇌병변 성인장애인을 돌보는 가족원의 돌봄부담의 내용과 정도를 파악하고, 장애인 돌봄가족원의 돌봄부담과 우울의 관계, 그리고 자살생각과의 관계를 알아보고자 수행되었다. 가족 간의 관계와 장애에 대한 사회적 지원이 돌봄부담이 돌봄가족원에게 미치는 부정적 영향을 완화하는 역할을 하는지 알아보고자 하였다. 이를 위하여 2017년 5～10월까지 서울‧경기지역 장애인복지관, 장애인자립생활지원센터, 특수학교 등 7개 기관의 이용자 212명에서 설문지를 배포하여 총 176명의 설문지를 회수하여 174명의 자료를 사용하였다. 분석은 SPSS 통계프로그램 21.0, Mplus 8 base program을 사용하였다.
주요 연구 결과는 다음과 같다.
첫째, 연구에 참여한 장애인 돌봄가족원의 돌봄현황은 16년 이상 주돌봄역할을 수행한 경우가 55.7%, 하루 평균 돌봄시간이 8시간 초과인 경우가 63%, 주 6일 이상 돌봄역할을 하는 경우가 83.6%로 지속적, 장시간 돌봄역할을 수행하고 있었다. 주관적 돌봄부담은 0-3점 범위에서 평균 1.74로 나타났고, 하위 영역으로는 일에 방해 받음이 2.15로 가장 높았고, 건강의 부담이 1.80, 경제적 부담 1.76, 가족지원의 부족 1.27의 순이었다.
둘째, 돌봄가족원의 우울 수준은 CES-D 평균 20.2점의 수준이었다. 돌봄가족원이 여성인 경우 우울수준이 20.7점으로 남성의 18.0점에 비해 높았다. 그리고 돌봄가족원의 지난 해 자살생각‧시도의 경험이 있는 비율은 24.9%로 나타났다. 역시 돌봄가족원이 여성인 경우 26.4%로 남성의 17.2%에 비해 높게 나타났다.
셋째, 연구 가설에 대한 검증 결과는 다음과 같다.
연구가설1 검증: 돌봄가족원의 주관적 돌봄부담은 우울을 매개로 자살생각에 영향을 미치는 것으로 나타났다.
연구가설2-1 검증: 돌봄가족원의 긍정적 가족관계태도가 주관적 돌봄부담과 우울의 관계에서 부적 방향으로 매개역할을 하는 것은 유의하지 않게 나타났다.
연구가설2-2 검증: 돌봄가족원의 주관적 돌봄부담이 긍정적 가족관계태도와 우울을 매개로 자살생각에 영향을 주는 3단계 매개효과는 유의하지 않게 나타났다.
연구가설3: 사회적 지원이 돌봄가족원의 주관적 돌봄부담이 우울을 매개로 자살생각에 영향을 미치는 관계에 대해 조절매개효과가 있는 것으로 나타났다.
구조방정식을 이용하여 연구가설 2-2와 연구가설 3의 모형을 제시하였으며 연구가설 2-2 모형에서 유의한 경로는 다음과 같다.

[경로1] 돌봄가족원의 주관적 돌봄부담→ 우울 (+)
[경로1] 돌봄가족원의 긍정적 가족관계태도→ 우울 (-)
돌봄가족원의 주관적 돌봄부담이 우울을 매개로 자살생각으로 가는 경로가 유의하게 나타나 돌봄부담의 부정적 영향이 검증되었다. 긍정적 가족관계태도는 돌봄부담에서 우울로 가는 경로에서 매개역할을 하지는 않으나 돌봄가족원의 우울을 유의하게 낮추는 영향이 있는 것으로 나타났다. 사회적 지원은 돌봄가족원의 주관적 돌봄부담이 우울을 매개로 자살생각에 미치는 영향에 대해 조절매개효과가 있는 것으로 나타났다.
본 연구 결과를 바탕으로 장애인 가족과 돌봄가족원에 대한 실천적, 정책적 함의를 다음과 같이 논의하였다.
실천적으로는 장애인 돌봄가족원의 정신 건강을 지원하는 프로그램의 개발, 장애인 가족에 대한 가족관계 프로그램의 개발과 확대를 제안하였고, 정책적으로는 장애인에 대한 돌봄서비스의 확대와 장애인 돌봄가족원의 휴식지원을 위한 돌봄서비스 도입과 확대를 제안하였다.
