초록[abstracts]
During a public health crisis, decision-making in terms of determining a hospital organization's priority resources—such as facility, information, workforce, and others—and assigning them is the core element of crisis management. In particular, for infectious diseases, the judgment made during the initial stage makes a significant difference in the size and severity of the ultimate crisis, and its influence manifests in the overall population, which calls for a more prudent decision-making. The Middle East Respiratory Syndrome(MERS) outbreak that occurred in South Korea in 2015 was a representative public health crisis that spread into a much larger area because of a decision-making failure.
However, since the said MERS outbreak, related studies have been mainly focused on the epidemiological characteristics of the disease or physical structure and elements such as policies, laws, and others. Therefore, in this study, the characteristics of the decision-making process in the hospital organization are explored, and the structural and contextual factors that affected them are analyzed. In particular, by applying the concept of “Accountability for Reasonableness” the decision-making process related to determining priorities is to be evaluated in the perspective of procedural fairness.
As such, by applying the qualitative research method, individual interviews were conducted with managers who had the decision-making power among the hospital workers during the 2015 MERS outbreak. In addition, white papers and publications by governments, hospitals, and academic societies were collected. These data were established as a database using NVivo 10 software, and they were analyzed using the framework analysis method.
Main issues that required the hospital's internal decision-making during the MERS outbreak were determining priority and assigning the resources in response to the surge in demand as well as continuation of facility operation, the trasfer of patients, and medical judgments such as medication, examination. The judgment bases that were applied during that process were many principles related to the organization's survival such as expected financial loss, relationship with other institutions, public image, and others, as well as work-related principles such as the duty of care patients and the prevention of the spread of infection.
In addition, as a result of evaluating the decision-making process by applying four conditions of “Accountability for Reasonableness”, the conclusion that there must be “a balance between authority and responsibility” among the frontline specialists and leaders at the site for a more rational and fairer decision-making process was drawn. As a structural element for its promotion, it was important to establish mutual trust through a transparent communication system with a secured feedback route.
This study was the first one to comprehensively explore the internal decision-making process of the hospital during the MERS outbreak in 2015 through the voices of the organization members, and it was meaningful that it did not limit the scope of infectious disease crisis management to an epidemiological perspective but extended it to the society, culture, environment, and others. In addition, it is expected that this study can be used as basic data for the establishment of a hospital organization's strategy in responding to a public health crisis, which can possibly occur again in the future.

공중보건위기 상황에서 병원조직의 장비와 시설, 정보, 인력 등 자원의 우선순위를 설정하고 배치하는 의사결정은 위기관리의 핵심 요소이다. 특히 감염병의 경우, 초기 판단의 성패여부에 따라 궁극적인 위기의 규모와 심각도가 달라지기며 그 파급력이 인구집단 전반에 걸쳐 나타나기 때문에 더욱 심중한 의사결정이 필요하다. 2015년 국내 발생한 메르스(MERS) 사태는 의사결정의 실패로 인해 대규모 감염으로 확산된 대표적인 공중보건위기였다.
그러나 사태 이후 메르스 관련 연구는 주로 역학적 관점에서의 질병의 특성이나 제도, 법률 등 물리적 구조와 요소에 치중하고 있다. 이에 본 연구에서는 병원 조직 내에서 이루어졌던 의사결정 과정의 특성을 탐색하고, 이에 영향을 미치는 구조적·맥락적 요인 분석하였다. 특히 ‘합리성에 관한 책무’ 개념을 적용하여 우선순위 설정 관련된 의사결정 과정을 절차적 공정성의 관점에서 평가하고자 하였다.
이를 위해 본 연구에서는 질적연구방법을 적용하여 2015년 메르스 사태 당시 병원 종사자 중 의사결정의 권한이 있었던 관리자를 대상으로 개인 심층면접조사를 시행하였다. 또한 지자체 및 개별 병원, 학술단체에서 발간한 메르스 백서 및 출판 서적을 수집하였다. 이상의 자료들은 NVivo 10 소프트웨어를 이용하여 데이터베이스로 구축되었으며 프레임워크 분석 방법을 적용하여 분석하였다.
공중보건위기 상황에서 병원 내부에서 의사결정 해야 하는 주요 사안으로는 진료시설 운영 지속 여부, 환자의 이송이나 투약, 검사와 같은 의학적인 판단뿐만 아니라 급격한 수요의 상승에 따라 턱없이 부족한 인력이나 물자와 같은 자원의 우선순위를 정하고 배치하는 것이었다. 이 과정에서 주로 적용한 판단 근거는 환자 치료에 대한 의무, 감염 확산 차단과 같은 직무에 근거한 원칙 외에도 예측되는 경제적 손익, 타 기관과의 관계, 대외적 이미지 등 조직의 생존과 관련되는 원칙들이 다수 고려되었다.
또한 AFR의 네가지 조건을 적용하여 의사결정 과정을 평가한 결과 불확실한 상황에서 보다 합리적이고 공정한 의사결정 과정을 위해서는 일선 현장의 전문가와 리더 사이에서의 ‘권한과 책임의 균형’이 이루어져야 한다는 결론이 도출되었다. 이를 촉진하기 위한 구조적 요인으로는 투명하고 피드백 경로가 확보된 의사소통 체계를 통한 상호신뢰 구축이 중요한 것으로 나타났다.
본 연구는 내부 조직원의 목소리를 통해 메르스 당시 병원 내 의사결정 과정을 포괄적으로 탐색하고자 한 첫 시도였으며, 감염병 위기관리를 역학적 측면으로 국한하지 않고 사회, 문화, 환경 등 확장된 범위에서 접근했다는 점에서 의의를 갖는다. 더 나아가 또 다시 발생할 수 있는 공중보건위기에 대응하기 위한 병원조직의 전략 수립에 기초 자료로 활용될 수 있을 것으로 기대된다.
