국문 초록[abstracts]

헤븐 프로젝트는 이탈리아 신경외과의 세르지오 카나베로(Sergio Canavero) 등이 시도하고 있는 머리 이식 수술이다. 헤븐 프로젝트가 현실화됨에 따라 대두되는 윤리적 문제를 다루기 위해 이 연구는 관련 논문의 키워드 분석 및 이슈별 분석을 통해 학술적 논의 현황을 종합 정리하였다. 키워드 분석 결과 어휘들이 transplantation을 중심으로 하는 군과 head transplantation을 중심으로 하는 군이라는 두 개의 군으로 분리된다. 이중 head transplantation 군은 기술과 관련된 어휘들과 주로 연결된다. 해악금지의 원칙에는 척수손상(Spinal Cord Injury, SCI), 대뇌허혈(Cerebral ischemia), 저체온증(Hypothermia)에 대한 임상시험의 안전성, 유효성 기준이 결여되어있고, 치료의 목표가 불명확하고 수술 가치가 비실재적이며, 성공의 합리적인 가능성이 없는 치료와 또한 자아 정체성과 성적 정체성 같은 이슈들이 포함되어 있다. 자율성 존중의 원칙에는 충분한 정보에 의한 동의가 사실상 어려운 이슈가 포함된다. 정의의 원칙에는 장기의 배분, 치료 비용이나 빈자 신체의 착취라는 사회적 격차 심화, 그리고 법적 주체의 문제 및 살인의 정의 등 법률적 문제 등의 이슈가 포함된다. 또한 연구 윤리에는 동물실험의 목적 왜곡, 동물자료 미공개, 장기적 생존통계 미비, 동료심사보다는 동영상이나 홍보자료 근거, 의도적인 인용 오류 등 이슈가 포함된다. 뇌중심주의의 문제도 지적된다. 문헌 분석에 바탕을 둔 이 연구결과는 향후 윤리문제에 대한 연구 및 정책 결정에 기초 자료로써 유용하게 사용할 수 있을 것 같다.

다국어 초록[Multilingual Abstract]

The Heaven Project is a head/body transplant surgery that is attempted by Sergio Canavero, an Italian neurosurgeon. In order to address the ethical problems that emerged as the Heaven Project became a reality, this study summarizes current academic debates based on key word analysis and issue-specific analysis of related papers. Keyword analysis results show that words are divided into two groups, one centered on transformation and the other on head transformation. The head transformation group is mainly linked to technology related words. The principle of prohibition of harm includes issues such as the lack of safety and efficacy standards of clinical trials on SCI, Cerebral ischemia and Hypothermia, unclear objectives of treatment, illusive values of surgery, no reasonable possibility of success, self-identity and sexual identity. The principle of respect for autonomy addresses issues that consent by sufficient information is in reality difficult to obtain. The principle of justice includes issues such as the widening social gap related with the allocation of organs, treatment costs and exploitation of the poor, and legal problems including legal agents and definition of homicide. Research ethics also include issues such as the distortion of the purpose of animal testing, the disclosure of animal data, the lack of long-term survival statistics, the lack of peer reviews, and intentional errors of citation. The problem of cerebral centrism is also pointed out. These findings, based on literature analysis, are likely to be useful as basic data for future research and policy decisions on ethical issues.
