초록[abstracts]

[본 연구는 최근 미국과 일본의 장기요양제도 관련 주요 개혁을 비교 분석하여 한국 노인장기요양 정책에 시사점을 제언하고자 양 국가의 개혁관련 문헌분석을 중심으로 실시되었다. 미국과 일본은 최근 ‘오바마개혁’과 ‘아베개혁’으로 불러진 의료 및 장기요양정책에 관한 주요 개혁을 추진하였다. 미국의 공적 장기요양 정책은 메디케이드와 메디케어의 장기요양지원에 관한 개정과 논란이 30년간 지속되어 오다가 오바마 대통령의 적정부담의료법통과 이후 일대 전환기를 맞게 된다. 일본은 2012년 이후 ‘의료․개호서비스 제공체계의 효율화, 중점화와 기능강화’를 실현하기 위해 노력하였고 마침내 2015년 ‘의료개호종합확보추진법’이 제정되었다. 분석결과 이러한 두 국가의 최근 개혁의 핵심적 특징은 지역사회 중심의 장기요양보호에 대한 지역성, 예방성 및 의료-요양 통합성 등에 있다. 본 연구는 첫째, 양 국가의 개혁에 대한 배경으로 장기요양 정책의 변천과정을 논의하고자 한다. 둘째, 본 개혁의 장기요양 관련 특징을 살펴보고자 하였다. 셋째, 양 개혁의 특징을 지역보호성, 예방성, 통합적 케어 정도의 틀에 맞추어 비교 분석하였다. 마지막으로 이러한 분석이 우리나라 장기요양정책에 주는 시사점을 고찰하였다.,

The purpose of this paper is to examine the comparison between the recent long-term care policies of both Japan and the US. Both countries has implemented the recent LTC policy reforms, so-called Abe-reform and Obama plan. Since 2012, Japanese LTC reform have focused on the efficiency, focalization, and enhancement of the medical and LTC service provision system. It turned out to enact the ‘Act of comprehensive improvision of integrated medical care and long-term care’ in 2015. In the US, public LTC policies has been long time issues derived from the debates of Medicaid, Medicare and other health care financing. The Obama government has tried to solve those issues by implementing ‘the Patient Protection and Affordable Care Act’. The results of this study shows the three core points of the recent LTC reforms of both countries, community-focusing, prevention and integrating health and long-term care. In the first section, this study discussed the history of both countries’ LTC policies. In the second section, by focusing on community-focusing, prevention and integrating health and long-term care, the study compared the main characteristics of recent reforms between both countries. Lastly, the study suggests several ways to reform the Korean LTC insurance policy. The implication of this study is believed to impact on planing of the future LTC insurance reform in Korea.]
