북한이탈여성의 피임인식 및 피임교육의 필요성

초록[abstracts]
최근 남한에 입국하는 북한이탈여성의 수가 지속적으로 증가하는 추세이며 관련 연구 또한 늘어나고 있다. 그러나 성과 관련된 주제는 예민한 사안으로 연구가 어려우며 특히 피임과 관련된 연구는 전무한 실정이다. 따라서 본 연구는 북한이탈여성들의 피임인식 및 경구피임약 복용경험을 파악하여 북한이탈여성을 대상으로 하는 피임교육의 필요성을 인식하고 그 방향을 모색하기 위한 기초자료를 제공하고자 하였다.
남한에 거주한 지 3년 이상 된 북한이탈여성 6명을 대상으로 1:1 심층 인터뷰를 진행하여 실제 피임에 대한 대상자들의 인식 및 경험, 교육에 관한 의견을 파악하였다. 질적연구 분석방법인 Thematic analysis를 통하여 인식 및 경험에 대한 유사점 및 차이점을 주제별로 분석하였으며 기존의 북한이탈여성 대상 성지식 연구결과와 비교하였다.
연구에 참여한 6명의 북한이탈여성은 연령별로 20대, 40대, 50대 각각 2명씩(33.3%)이었으며 결혼상태는 미혼, 기혼 각각 3명(50%)이었다. 탈북 이전에는 임신 및 출산, 피임 교육이 전무하였고 피임법 또한 기혼여성을 대상으로 하는 고리(루프)가 유일하였으며 피임에 대한 지식부족은 원치 않는 임신으로 이어져 낙태가 성행하였다. 탈북 이후 중국에서 3년 이상 체류한 북한이탈여성은 3명으로 중국에서 피임에 관한 새로운 정보를 습득하였으나 정보의 정확도가 떨어지고 경험했던 피임법에 대한 보건관리자의 충분한 설명 부재로 피임에 대한 이해는 여전히 부족하였다. 남한 입국 후 북한이탈여성들은 모두 검진, 치료, 피임을 목적으로 산부인과를 방문하였고 성교육을 받은 경험이 1회 이상 있었으며 새로 알게
된 피임에 관한 정보가 존재하였다. 연령대별로 20대의 경우 다른 연령대의 대상자들에 비해 피임에 관한 새로운 정보의 양이 많고 그 정보의 정확도가 높았다. 연구에 참여한 북한이탈여성 중 1명만이 경구피임약 복용경험이 있었으며 생리주기조절을 목적으로 경구피임약을, 피임을 목적으로 사후피임약을 복용하였다. 북한이탈여성들은 피임 중심의 성교육 요구도가 높았으며 주변의 원치 않는 임신 및 낙태 사례를 통하여 북한이탈여성들을 대상으로 하는 피임교육의 절실함을 호소하였다. 그 방법으로는 강의 형식을 가장 선호하였으며 최소 3회 이상 진행하여 교육에 대한 이해도를 높이기 원했으며 이후 필요 시 1:1 상담을 선호하는 것으로 나타났다.
본 연구결과를 바탕으로 북한이탈여성의 피임지식 수준을 높일 수 있는 교육프로그램 개발과 이를 위한 정부 및 보건관리자들의 역할 모색에 대한 필요성을 제언하였다.
초록[abstracts]
Recently, the number of North Korean defector women entering South Korea has continuously increased as well as the related research. However, considered as a sensitive issue, sex-related topics are difficult to research, especially those researches related to contraception being almost none existent. Therefore, this research is to provide the basic data to realize why contraception education is necessary for the defectors, and how to grope for the direction by understanding their perceptions toward contraception, and their experience with the oral contraceptive pill.
We conducted an in-depth interview toward 6 defector women who have stayed in South Korea for more than three years to find out their perceptions, experience, and education about contraception. We analyzed them based on each subject through ‘Thematic analysis’, one of qualitative researches, and compared it with the existing research results about knowledge of sexual matters of North Korean defector women.
The 6 defector women who participated in the research were composed of two in the 20s, two in the 40s, and two in the 50s (33.3%). The marital status was three single and three married (50%). Before escaping from North Korea, they had never been educated about pregnancy, childbirth, or contraception. In regards to the way of contraception, Intrauterine device was the only contraception method they knew about, which was for the married women. Such lack of knowledge about contraception led to the unwanted pregnancy, which eventually made abortion prevailing among them. The three people, who stayed in China more than 3 years after the escape, achieved the new information about contraception but it lacked accuracy. Also, they still lacked understanding about the subject due to the lack of enough explanation from health administrators. After arriving in South Korea, the North Korean defector women all went to see gynecologist for health inspection, treatment, and contraception. It appeared that they had received contraception education more than once, and had the contraception information they newly became aware of. Defector women in their 20s were more aware of the new information about contraception, and the knowledge was more accurate than others in different age group. Only one among the total 6 defector women had experience having oral contraceptive pill for adjusting the menstruation cycle, and the postcoital contraception. They had high demand about sex education with focus on the contraception, and eagerly appealed for the contraception education for the North Korean defector women as there were many cases of unwanted pregnancy and abortion around them. As a way, they most preferred the lecture type, minimum of 3 times so that they could have a better understanding about the contraception. Also it appeared that they prefer one on one counseling if needed after having the lectures.
Based on this research result, it is proposed that developing an educational program which can increase the awareness of the North Korean defector women toward contraceptive knowledge is necessary, and the government and health administrators need to examine their roles.
