임상시험실시기관의 내부점검을 통한 임상시험 품질관리에 대한 고찰

초록[abstracts]
The purpose of this study This study reviews the case of the Dana-Farber Harvard Cancer Center as an advanced case of quality management through internal audit and define the current status and problems of institution(clinical center) in South Korean in order to find ways to improve the internal audit system. The methods of this study This study reviews the case of Quality Assurance Office for Clinical Trials at the Dana Farber Medical Center. And, to investigate the status of internal audit of clinical trial in South Korea, this study conducted a survey of 31 internal auditors in 18 hospitals. The review of the advanced case The Quality Assurance Office for Clinical Trials at the Dana Farber Medical Center has a internal audit system since 1988, and it is detailed and systemic to support and manage the clinical trials. The internal audit process is thought be very fair, accurate, and precise. The conclusion of this study The review of the advanced case and the results of a survey of are summarized as follows: (1) Institutions in Korea do not have comparatively adequate system to support for quality management including internal audit. To improve this matter, the expansion for related organization and staffs is needed to develop the quality of clinical trials. (2) This study found that there is not enough staff and low level of competence to perform internal audit. To solve this problem, institutions need to supplement the personnel and internal auditors need more training which they can share their information internally and externally between institutions related quality management in clinical trials. (3) This study found that there should be the detailed methods and systems to evaluate the result of internal audit for quality management. Institutions need to expand their systems and organizations for more reliable results and reviews of evaluation in internal audit. (4) This study found that staffs in hospital have are not aware of the importance of internal audit in clinical trials. In order to raise their awareness, hospitals should engage their medical staff in developing the quality management system. The systemic process of internal audit and a mature approach to clinical trial can be developed based on this kind of participation.
초록[abstracts]
연구의 목적 임상시험 내부점검에 대한 외국의 선진 사례로서 Dana-Farber Harvard Cancer Center의 내부점검을 통한 품질관리 시스템을 검토하고, 한국에서의 현재 내부점검 상황과 문제점을 진단하여, 임상시험 실시기관의 내부점검의 성숙된 증진 방안을 제시하는 것이 본 연구의 목적이다. 연구의 방법 연구의 내용은 선진화된 사례로서 Dana Farber Medical Center 의 Quality Assurance Office for Clinical Trials의 내용을 사례 연구의 방식으로 검토하였다. 한국에서의 임상시험 내부점검의 현황을 알기 위하여 내부점검담당자에게 설문 조사를 실시하여, 18개 병원의 내부점검담당자인 31명이 본 연구를 위해 설문에 응답하였다. 선진사례에 대한 검토 및 평가 Dana-Farber Harvard Cancer Center 의 Quality Assurance Office for Clinical Trials의 내부점검 업무는 1988년부터 시작되었고, 임상시험을 협조하고 관리하는 체계가 굉장히 자세하고 체계적으로 갖추어져 있음을 확인할 수 있었다. 무엇보다 내부점검 결과를 평가하는 절차가 세분화되어 있어 공정하고, 정확한 평가가 가능한 체계가 구축되어 있다. 연구의 결론 및 고찰 선진사례에 대한 조사와 한국의 임상시험 실시기관의 설문지조사를 통해 나타난 것은 아래의 내용과 같다. (1) 한국에서의 임상시험 내부점검 및 임상시험 품질관리를 위한 병원 내 조직 체계 구조가 아직은 미흡한 것을 조사되었는데, 이를 개선하기 위해서는 임상시험 품질 향상을 가져올 수 있는 부서 및 조직의 확충이 필요하다. (2) 내부점검을 담당하는 인원의 수가 부족하고 담당자의 역량 강화가 필요한 것이 확인되었는데, 이는 담당자의 인원 충원 및 임상시험 품질관리와 관련된 내·외부 업무 관련자의 정보 공유 등을 통한 교육이 필요함을 시사한다. (3) 내부점검의 점검 결과를 결정하고 평가하는 과정에서 세분화된 절차와 체계가 필요함을 본 연구를 통해 확인할 수 있었는데, 이는 신뢰성 높은 점검 결과 및 평가를 위해 이를 검토하는 병원 내 체계 및 조직의 확충이 필요함을 의미한다. (4) 임상시험 내부점검에 대한 인식이 병원 내에서 전반적으로 낮은 것으로 확인되었는데 이는 병원의 임상시험 품질관련 체계를 설립하는 데 있어서 병원 내부의 연구자들과 함께 이를 함께 구축하여, 참여도를 높이고 그 중요성을 깨닫는 것이 필요다고 여겨진다. 이러한 내부점검의 체계화된 과정과 성숙된 임상시험 문화는 이러한 참여 속에서 발전될 것으로 기대된다.
